Safe and Sound Home Care Partners with PocketRN to Support Families Through Medicare’s GUIDE Model

Safe and Sound Home Care is proud to participate in Medicare’s Guiding an Improved Dementia Experience (GUIDE) Model through its partnership with PocketRN.
This program represents an important step forward in providing comprehensive support for individuals living with dementia and the family members and unpaid caregivers who support them.

The GUIDE Model is designed to help people living with dementia remain safely at home while improving quality of life for both individuals and their families. Through this program, eligible Medicare beneficiaries can receive support such as care management, access to a care team, respite services, and connections to valuable community resources.

Safe and Sound Home Care is committed to providing compassionate, professional in-home care that helps older adults maintain dignity, comfort, and independence at home. The company currently serves families through offices in Oxford, Tupelo, and Hernando, Mississippi.

What is GUIDE?

Medicare’s GUIDE Model provides comprehensive dementia care and caregiver support for eligible individuals with Traditional Medicare.
This is more precise since GUIDE has specific eligibility requirements.The goal of the program is to support family and unpaid caregivers of people living with dementia with the goal to enable people with dementia to safely remain in their homes and improving quality of life for both the person with dementia and their families. GUIDE will provide caregivers with tools, such as supported care management, to address the needs of both the caregiver and the person with dementia, along with 24/7 access to your care team, respite services, and community resources.

Who is eligible for GUIDE?

GUIDE is available for individuals who fall under the following criteria:

  •  Client has dementia (PocketRN can help obtain a diagnosis if appropriate.)
  • Client is enrolled in Original Medicare Parts A and B as their primary insurance (Cannot be enrolled in Medicare Advantage, including Special Needs Plans/Pace Programs)
  • Client not currently enrolled in hospice
  • Clients who live at home (not residing in memory care, nursing homes, assisted living, etc. Temporary rehabilitation stays and independent living are allowed)

Comprehensive Support for Patients and Caregivers

One of the most impactful aspects of the GUIDE Model is its holistic approach. Dementia doesn’t just affect the individual it deeply impacts families and caregivers as well. Recognizing this, the program offers a wide range of support services, including

Eligibility

  • Has dementia. PocketRN can help obtain a diagnosis if appropriate.
  • Has Traditional Medicare Parts A and B as primary insurance and is not enrolled in Medicare Advantage.
  • Is not currently enrolled in hospice.
  • Lives at home. Individuals residing in memory care, assisted living, nursing homes, group homes, adult family homes, or other residential care settings are not eligible. Temporary rehabilitation stays and independent living are allowed.
  • VA patients, individuals who are dually eligible for Medicaid, and individuals receiving palliative care may be eligible if they otherwise meetGUIDErequirements.

Personalized Care Coordination

Each participant receives a tailored care plan designed to address their medical, emotional, and daily living needs. This coordinated approach helps connect patients and caregivers with appropriate clinical and community-based support.

Caregiver Support and Education

Caregivers are provided with training, guidance, and resources to help them manage the complexities of dementia care. This support empowers families to provide better care while reducing stress and burnout.

Current Respite Benefit
“Qualifying families can receive $2,625 in annual respite care services—equivalent to a maximum of 76 hours of in-home care for the period July 1, 2026 through June 30, 2027. This respite benefit renews annually on July 1 and may vary from year to year.”
