Safe and Sound Home Care is proud to partner with PocketRN, a participant in Medicare’s Guiding an Improved Dementia Experience (GUIDE).
What is GUIDE?
GUIDE is a comprehensive dementia care model available at no-cost to Medicare beneficiaries. The goal of the program is to support family and unpaid caregivers of people living with dementia with the goal to enable people with dementia to safely remain in their homes and improving quality of life for both the person with dementia and their families. GUIDE will provide caregivers with tools, such as supported care management, to address the needs of both the caregiver and the person with dementia, along with 24/7 access to your care team, respite services, and community resources.
Who is eligible for GUIDE?
GUIDE is available for individuals who fall under the following criteria:
- Client has dementia (PocketRN can help obtain a diagnosis if appropriate.)
- Client is enrolled in Original Medicare Parts A and B as their primary insurance (Cannot be enrolled in Medicare Advantage, including Special Needs Plans/Pace Programs)
- Client not currently enrolled in hospice
- Clients who live at home (not residing in memory care, nursing homes, assisted living, etc. Temporary rehabilitation stays and independent living are allowed)
